Add not executed methods and namespaces

Hi Grant,

NCover seems to ignore methods and even namespaces if they were not executed within any test. This may lead to wrong coverage results. Please refer to my NCover post.

Would it be possible to optionally reflect the assemblies before displaying them? In this case we would result in correct coverage numbers.
